Position Overview:

Join Bluestone as an Administrative Associate, Care Coordination where you will work with the UCare SNBC (Special Needs Basic Care, 18-64 years of age) population throughout the Polk County, MN areas with members living in the community and group homes. In this role you will support a Bluestone Care Coordinator in managing their client’s designated health and Medicare services. This is a great opportunity to work with a multi-disciplinary healthcare team while gaining valuable experience in the healthcare field. This role requires excellent collaboration, organization, and strong customer services skills as well as the ability to work independently.

Responsibilities:

• Responsible for providing administrative support to a Care Coordinator, RN or LSW
• Assist in identifying resources and services such as medical equipment, social services, and county resources
• Manage referral process
• Ordering DME
• Complete required documentation and forms
• Telephone customer service, responding to member needs
• Scheduling appointments
• Assisting members throughout hospitalization and discharge
• Outreach - telephone calls to members
• Local travel (driving throughout the Polk County, MN areas)

Qualifications:

Education/Certification/Experience

• Previous experience as a HUC or completion of a Health Unit Coordinator program or Medical Secretary program preferred.
• Must have valid MN driver's license

Knowledge/Skills/Abilities

• Understanding of community resources
• Knowledge of health plan benefits and Medical Assistance preferred
• Familiar with medical terminology preferred
• Comfortable navigating electronic medical record systems and working with patient care notes
• Detailed oriented and accurate
• Excellent interpersonal and customer service skills
• Ability to problem solve and meet deadlines
• Computer proficient
• Demonstrated ability to read, write, speak, and understand the English language, bilingual candidate a plus
