Job Description

Our Home Health nurses are an integral part of the care management team, providing the important clinical support so vital to thrive in the place our seniors call home.

The Home Health RN functions as a case manager and develops and implements a home health plan of care that meets each patient's specific needs. The RN uses the nursing process, which includes the use of technology, therapeutic intervention, evidence-based practice and coordination of care with other health team members to achieve positive clinical outcomes with our home health patients.

Education and/or Experience:

Graduate of approved Registered Nurse School of Nursing and one year of clinical nursing experience. Current Cardio Pulmonary Resuscitation (CPR) certification required, or we will provide the training.

Certificates, Licenses, Registrations:

Possession of a valid RN license issued by the State of Pennsylvania. Maintains CPR certification per policy. Valid driver’s license and reliable transportation that is insured in accordance with Pennsylvania requirements.
