I. Job Summary:
Provide services to patients and their families while on the inpatient unit. Ensure that the clinical record includes a comprehensive social history of the patient, focusing on issues that directly affect treatment on the unit and the development of a successful discharge plan. Participate in the interdisciplinary team discussions and development of the treatment plan. Provide coordination/liaison with community-based social and mental health agencies to facilitate communication, discharges and coordinated care. Complete necessary assessments/applications to qualify the patient for needed services. Provide skill building groups to improve patients' understanding of mental illness and develop coping skills. Must function in conformance with organizational policies, procedures, and ethical standards as well as local, state and federal regulations.
II. Reporting Relationships:
A. Responsible to: RN-Inpatient Coordinator/Charge Nurse
B. Clinical Supervisor: Manager of Inpatient Social Work Services
C. Workers Supervised: None.
D. Interrelationships: Works closely with other members of the treatment team. Has contact with other departments throughout the organization.
E. Age of Client: Must understand the specific communication, development, and treatment needs of a wide range of age groups.
III. Qualifications (Knowledge and Critical Skills):
A. Professional:
1. Bachelor's degree in social work with experience in mental health treatment - BSW Required.
2. Ability to understand all Federal, State and Local regulations that are applicable to the practice of social work.
3. Has or is willing to obtain CPR Certification.
4. Has or is willing to obtain CPI Certification.
5. The ability to speak and hear or read lips is essential to communicate with clients, referral sources, peers, and others.
6. Must be able to work full time if hired as full-time employee.
B. Personal
1. Excellent communication and public relations skills.
2. Ability to work under high stress with multiple priorities.
3. Ability to utilize a high degree of independent judgement while collaborating with patients and families.
4. Must maintain a high level of objectivity when dealing with all issues encountered.
5. Able to uphold professional standards as a representative of the organization.
IV. Essential Functions of the Position:
1. Must attend and participate in client rounds every day. Position will work Thursday-Monday or Friday- Tuesday to provide weekend services to patients.
2. Work with treatment team, client, and family to develop and implement a timely discharge plan focusing on safety, stability, and continuity of care.
3. Promote good relationships with other agencies and departments.
4. Attend and /or participate in mandatory in-services and educational trainings.
5. Complete documentation and paperwork in a timely manner as per Center procedures.
6. Competent to complete psycho-social reviews, skills building groups and assessment tools, as appropriate i.e., ANSA, CANS, and DMHA Safety Plans
7. Ability to be self-directed in managing and coordinating services within the team and the agency.
OTHER:
1. Must be able to lift 50 pounds and have agility to assist with physical management.
V. Competencies:
Critical Thinking
1. Must be able to develop and update treatment plan as to the client’s changing needs.
2. Must be able to meet with the client and the client’s family and/or significant other to develop discharge plans for the client.
3. Must be able to recognize changing behaviors in clients and report them to the appropriate staff.
Interpersonal
1. Must have a good working relationship with the treatment team.
2. Must be able to work with other departments to develop discharge plans for clients.
3. Competent in building a therapeutic relationship while maintaining professional boundaries.
Technical
1. Ability to use computer and additional functions (like a signature pad, printer, scanner, etc.)
2. Ability to maintain HIPAA compliant records in an electronic medical record platform.
3. Ability to use basic functions on a telephone.
VI. OSHA
Category II- Understand that tasks performed may involve occasional exposure to blood and body fluids or tissues.
Following blood safety and other communicable disease precautions.
