Job Title: Home Health Admission Nurse (RN)

Schedule: Full-Time or Per-Diem Availability

Compensation: $150 per visit + mileage reimbursement

OASIS RN Responsibilities:

• Perform Admission / "Start of Care" OASIS Visits throughout an area of the Metro

• Evaluate and assess clients for home health services and create their plan of care

• Recommend services from other disciplines that have not yet been ordered by the physician

• Collaborate with clients, families, physicians, and other members of the home care team

OASIS RN Qualifications:

• Licensed as a Registered Nurse in the state of Minnesota

• Driving is required; must have a reliable vehicle with current insurance and a valid Driver's License

• Prior Home Health Nursing experience

• Proficiency with Medicare & OASIS documentation

• Demonstrated ability to work autonomously

• Excellent at planning, organizing, and communicating

Job Types: Full-time, Part-time, Per diem

Pay: From $150.00 per hour
