This includes medication-assisted treatment; outpatient substance use treatment; group and individual counseling; play therapy; psychiatry; parenting classes; overdose prevention training; vocational/educational counseling; primary care services such as: internal medicine, pediatrics, prevention and treatment of HIV, Hepatitis C treatment, hypertension, diabetes and asthma, and health education; and access to our free playroom.

CCHP is committed to providing our patients with the highest quality of care. We are a Patient Centered Medical Home (PCMH), licensed by the Office of Addiction Services and Supports (OASAS) and accredited by the Commission on Accreditation of Rehabilitation Facilities (CARF).

CCHP is seeking a Nurse Practitioner to join our multidisciplinary team. The Nurse Practitioner will provide comprehensive care to patients in our primary care program.

ESSENTIAL DUTIES AND RESPONSIBILITIES

· Provide medical care in line with training,

· Responsible for comprehensive recording of work-up activities, diagnostic findings, medical plans and treatment follow up.

· Order, interpret and evaluate diagnostic tests as appropriate

· Prescribe or administer treatment, medications, immunizations and other specialized medical care to treat or prevent illness, disease, or injury.

· Participate in multidisciplinary case reviews and family treatment in conjunction with other Center staff to formulate comprehensive treatment and rehabilitative plans.

· Participate in the development and implementation of new services, trainings and quality improvement projects

· Participate in peer review of medical chart notes and other quality assurance measures as directed.

· Assist in community outreach and other programs concerning prevention or treatment of injury or illness.

· Provide tele-health services, and on-call coverage as needed

EDUCATION AND EXPERIENCE

· Computer literacy and EHR experience required

· Experience working in a community health clinic or practice preferred

· Valid New York State Registered Nursing License, DEA, CLS and BLS certification

· Ability to work effectively with diverse populations

· Commitment to the mission of CCHP

Job Type: Full-time

Rate: $60.00-62.00 p/h
