Summary
Provides occupational therapy services to the participants of the AllCare PACE program and is part of the interdisciplinary team at the AllCare Pace building. This position requires some travel to patient's home.

Essential Duties

• Performs initial and annual screenings/assessments as assigned and makes appropriate recommendations for services with special focus on improving or maintaining independence and safety.
• Maintains regular attendance and participates in interdisciplinary team meetings; collaborates on care planning; and service allocation decisions.
• Completes participant evaluations as needed and develops a plan of care. Areas of evaluation include and are not limited to: functional, physical and activities of daily living (ADL) status, cognitive functioning, visual-perceptual status, and feeding ability, cooking skills, bed mobility, wheelchair needs, home safety/environmental assessments and restraint assessments.

Education & Experience
Bachelor of Science degree in Occupational Therapy from an accredited program and current certification with the State of Oregon. A minimum total of one year of personal, clinical/internship, and or employment experience providing care or services for adults with disabilities or frail elderly population required.

Certificates, Licenses and/or Registrations
Current BLS CPR certification and Driver’s license
