Monday - Friday Schedule, Daytime Hours

$5,000 Sign-On Bonus for qualified hires certified in Lymphedema

The Occupational Therapist - Lymphedema (OT) provides Lymphedema evaluations and treatment plans to participants. All participant care provided is in accordance with applicable InnovAge policies and state regulations. The Lymphedema OT works in collaboration with the Float Team Therapy Supervisor for purposes of scheduling care for the participant in addition to communicating with the Lymphedema team and Primary OTs regarding the Lymphedema needs of participants.

  • Provides clinical Lymphedema evaluation and treatment on an acute, restorative and maintenance level to program participants.
  • Assesses condition, creates a treatment plan and discusses implementation of therapy with participant or caretaker(s).
  • Integrates Lymphedema therapy treatment plan in the team Plan of Care (POC).
  • Assists in the ordering, inventory, distribution, and maintenance of durable medical equipment and supplies for participants.
  • Makes home visits/educational sessions as needed and indicated in the POC, and documents activities including participant/caregiver response in participant’s chart.
  • Educates participants and family members in proper care and use of various lymphedema supplies/DME.
  • Administers treatment program per referral from assessments from the interdisciplinary team (IDT)
  • Instructs family members, caregivers, and staff in methods to maximize safety and functional abilities in performing Activities of Daily Living (ADL) with documentation in medical record notes.
  • Documents Lymphedema therapy services provided and performs department record-keeping in keeping within documentation requirements.
  • Assists the physician or intermediate care provider in evaluating level of function, helps develop the plan of care and revise as necessary.
  • Writes reports to physicians regarding patient's progress.
  • Completes a discharge summary upon completion of therapy services.
  • Participates in utilization review of medical records as assigned.
  • Work in collaboration with InnovAge nursing home and assisted living staff regarding Lymphedema needs of participants who are their residents.
  • Attends Morning Meetings, mini teams and family meetings and forwards all information to the Primary OT if no other OT is available or if assigned by the Primary OT
  • Participates within the interdisciplinary team in the formulation of Plans of Care for InnovAge PACE program participants, as well as in other interdisciplinary team settings that plan, coordinate and monitor the care of InnovAge PACE program participants.

REQUIRED

  • Graduate of an Occupational Therapy curriculum accredited jointly by the Committee on Allied Health Education and Accreditation of the American Medical Association and the American Occupational Therapy Association (AOTA)
  • Bachelors in (OT), Masters in OT (MOT), Doctorate in OT (OTD)
  • National Certification by the American Occupational Therapy Association or eligible for National Registration Examination of the American Occupational Therapy Association.
  • Registration with state in which practicing
  • Requires state issued driver’s license, personal transportation, good driving record and auto insurance as required by law
  • A minimum of one year experience working with the frail or elderly is required.
  • Graduate of Certified Lymphedema Therapy program and credentialed as CLT with eligibility to sit for LANA exam

PREFERRED

  • Two (2) years’ experience, preferred
  • A minimum of one year of general occupational therapy experience in a geriatric setting preferred

The pay offered for the position will take into consideration the candidate’s geographic region, job-related knowledge, skills, experience, and internal equity, among other factors.
