Job Description :

We are an independent, not-for-profit visiting nurse agency in RI and we’re growing! We’re looking to add more positive, hard working, reliable, and experienced professionals to our team! We are searching for a per diem occupational therapist to service patients in the field based from our Portsmouth, RI home office.

  • Under administrative direction of the Rehabilitation Director and Supervisors, provides occupational therapy evaluations and provides appropriate treatment and instruction to patients and families.
  • Provides consultation and in-service education to nursing and agency staff as requested.

EDUCATION & EXPERIENCE:

  • Graduate of a fully accredited baccalaureate program in occupational therapy.
  • Current certification by the American Occupational Therapy Association and licensed by the State of Rhode Island and Massachusetts.
  • Two years of occupational therapy experience is preferred.

JOB DUTIES:

  • Determines priority needs for occupational therapy service depending on potential for benefit, diagnostic category, age, chronicity of condition, and availability of service.
  • Initiates care on referral and with written orders from physician, hospital, rehabilitation department or other referral sources as appropriate.
  • Performs admission assessment in accordance with agency paperwork policy.
  • Performs occupational therapy evaluation and obtains physician's orders to treat patient.
  • Establishes treatment goals and plan of care.
  • Gives direct treatment which may include therex, ADL training, transfer training, cognitive retraining, UE prosthetic training, and instruction in functional activities.
  • Plans and suggests adaptive equipment and adaptations to the home environment to assist the patient to function at optimum level.
  • Plans, instructs, and supervises patient or family member in carrying out the occupational therapy program in order to achieve highest level of function.
  • Instructs and acts as a consultant to other health team personnel in aspects of rehabilitation in which they may work with individual patient.
  • Reports patient progress and any significant change in condition to physician.
  • Obtains written verification from physician for continuation of treatment every two months or more often depending on the needs of the patient.
  • Keeps written records of treatment utilizing outcome planners.
  • Discharges patient when maximum benefit has been received. Sends a written discharge summary to the physician.

QUALIFICATIONS:

  • Knowledge of the principles, practices, and modalities of occupational therapy.
  • Knowledge of occupational therapy equipment and materials and their source of supply.
  • Knowledge of community resources including community rehabilitation agencies. Ability to aid the patient to achieve and maintain a maximum level of self-care independence.
  • Ability to keep accurate records and to write concise reports.
  • Ability to work in a cooperative relationship with other disciplines in achieving goals of the program.

  • Currently servicing primarily home care & hospice & palliative patients in the EAST & WEST service area territories including the following areas of RI: Jamestown, Prudence Island, Newport, Middletown, Portsmouth, Tiverton, Little Compton, Bristol, Warren, Barrington, and Riverside, RI. May occasionally see patients in other parts of RI currently including: East Providence, Pawtucket, North Providence, Providence, Johnson, Cranston, West Warwick, Warwick, Coventry (not including Greene), East Greenwich, North Kingstown, and Saunderstown, RI
  • Also, currently servicing some home care and hospice & palliative care patients in southeastern MA including: Swansea, Somerset, Fall River, Adamsville, & Westport, MA. May occasionally see patients in other parts of southeastern MA currently including Seekonk and Rehoboth, MA
