Job Description :

The Pharmacist – Program Specialist provides direct patient care services for patients with chronic illnesses, promote health and wellness, and participates in innovative patient visits including transitions of care, patient education classes and shared visits.

Compensation: $120,000 - $150,000

Essential Functions

  • Practices pharmacy in compliance with all governing regulatory authorities
  • Reviews patient profile to ensure appropriate drug selection and dosage
  • Dispenses medications and maintain accurate inventory records as required by appropriate regulatory authorities following all procedures defined by the pharmacy
  • Provides accurate, adequate, drug information/education to patients, families, and providers on drug interactions, side effects, dosage, and storage of pharmaceuticals
  • Assesses medication adherence during the process of medication counseling
  • Demonstrates the ability to set priorities, work independently and cooperatively with others within and outside the department
  • Present information (verbal and/or written) in a clear and concise manner
  • Provides pharmaceutical care services through direct interactions with patients and providers and work independently under individualized scope of clinical practice
  • Conducts face-to-face or virtual care to provide chronic disease state medication management services
  • Obtains comprehensive medical and/or medication histories from patients and/or their records and summarizes the significant findings as documented in the medical record to ensure safe and effective careDesigns, implements, assesses, monitors, and documents therapeutic plans utilizing the most effective, least toxic and most economical medications under scope of practice
  • Initiates, modifies, renews, refills, or discontinues medication therapy using approved delegated procedures
  • Orders labs and other tests to help determine efficacy and toxicity of medication therapy using approved delegating procedures


Education:

  • Graduate of an accredited college/university of pharmacy (Pharm.D or B.S. with equivalent experience) required
  • Completion of ASHP accredited PGY-1 Pharmacy Residency required
  • Certification in immunization delivery, smoking cessation, and naloxone (or eligible) required
  • Valid license from the Board of Pharmacy of the State of Texas required
  • Certification in Basic Life Support / First Aid / CPR required
  • Board Certified Pharmacotherapy Specialist (BCPS) or Board Certified Ambulatory Care Pharmacists (BCACP), preferred
  • 2+ years of community pharmacy experience preferred
  • 2+ years of medication adherence counseling experience preferred
  • Experience in HIV/AIDs, Geriatrics, and/or Pediatrics preferred

  • Completion of ASHP accredited PGY-1/2 Pharmacy Residency in Ambulatory Care, Adult Medicine, Geriatric Medicine, or Pediatrics preferred
  • 2+ years of healthcare experience required
