Our Community Health Organization is seeking a Pharmacist to work within a multi-disciplinary team to provide exceptional integrated care at our Community Health. Performs prescription processing and verification, patient counseling, and performs drug utilization reviews of patient profiles to prevent drug-drug interactions, drug-disease interactions, and duplicative therapies, and to increase medication adherence for the ages of clients served (0-adult).

Compensation: $120,000 - $150,000

Education:

• Graduate of an accredited college/university of pharmacy (Pharm.D or B.S. with equivalent experience)
• Current Texas Pharmacist licensure in good standing (or eligible)
• Certification in immunization delivery, smoking cessation, and naloxone (or eligible)
• Computer experience – Windows, Excel, and Accounting software.
• Post-graduate pharmacy residency preferred
• Must maintain First Aid /CPR
• Spanish speaking preferred

Work Experience Requirements

• 2+ years of healthcare experience, required
• 1+ years of community pharmacy experience, preferred
• 1+ years of medication adherence counseling experience, preferred
• 1+ y ears of Medicare market experience, preferred
• Experience in HIV/AIDs, Geriatrics, and/or Pediatrics, preferred
