Seacoast Nursing and Rehabilitation is seeking a Licensed Social Worker (LSW) 32 hours per week to join our fantastic team! This is an exciting opportunity to work at this highly sought-after BaneCare skilled nursing facility. Come join an amazing team! Requirements for Licensed Social Worker (LSW):

• Must possess a minimum of a B.A in Social Worker, LSW or equivalent degree with a current social work license.
• Nursing home experience preferred

• Job Duties of License Social Worker (LSW):

• Complies social history data
• Participates in assessments, development and evolution of residents care plan.
• Responsible for planning and leadership interdisciplinary care planning weekly meetings.
• Documents timely progress notes according to state and federal regulations.
• Coordinates the discharge planning for residents.
• Able to interpret and implement established social service policies, procedures and regulations.
• Coordinates services of community agencies for continued delivery of care upon discharge.
